Complications of regional citrate anticoagulation: accumulation or overload?
نویسندگان
چکیده
Regional citrate anticoagulation (RCA) is now recommended over systemic heparin for continuous renal replacement therapy in patients without contraindications. Its use is likely to increase throughout the world. However, in the absence of citrate blood level monitoring, the diagnosis of citrate accumulation, the most feared complication of RCA, remains relatively complex. It is therefore commonly mistaken with other conditions. This review aims at providing clarifications on RCA-associated acid-base disturbances and their management at the bedside. In particular, the authors wish to propose a clear distinction between citrate accumulation and net citrate overload.

Regional citrate anticoagulation for slow continuous ultrafiltration: risk of severe metabolic alkalosis
BACKGROUND Slow continuous ultrafiltration (SCUF) is a safe and efficient treatment for fluid overload in patients who are hemodynamically unstable, have low urine output, and are not in need of dialysis or hemofiltration for solute clearance. Sustained anticoagulation is required for these long treatments, thus posing clinically challenges for patients having contraindications to systemic anti...
متن کاملCitrate Anticoagulation for Continuous Renal Replacement Therapy in Critically Ill Patients: Success and Limits
Citrate anticoagulation has risen in interest so it is now a real alternative to heparin in the ICUs practice. Citrate provides a regional anticoagulation virtually restricted to extracorporeal circuit, where it acts by chelating ionized calcium. This issue is particularly true in patients ongoing CRRT, when the "continuous" systemic anticoagulation treatment is per se a relevant risk of bleedi...
متن کاملIon-exchange resin anticoagulation: experimental data of a novel extracorporeal regional anticoagulation technique
Introduction Regional anticoagulation has been introduced to avoid the complications of systemic anticoagulation. The most diffuse technique for regional anticoagulation is the infusion of citrate, which exerts its anticoagulative effect by chelating calcium, an essential factor of coagulation. Despite being a “regional” anticoagulation technique, a major fraction of the infused citrate reaches...
متن کاملContinuous venovenous hemofiltration with or without predilution regional citrate anticoagulation: a prospective study.
BACKGROUND/AIMS Continuous venovenous hemofiltration (CVVH) requires anticoagulation to prevent circuit clotting and its use is contraindicated in patients with high bleeding risk. The aim of this study was to compare CVVH with and without regional citrate anticoagulation (RCA) with respect to filter life, azotemic control and cost. METHODS This was a prospective sequential cohort study. The ...
متن کاملLong-term continuous renal replacement therapy and anticoagulation with citrate in critically ill patients with severe liver dysfunction
BACKGROUND As of 2009, anticoagulation with citrate was standard practice in continuous renal replacement therapy (CRRT) for critically ill patients at the University Medical Centre of Saarland, Germany. Partial hepatic metabolism of citrate means accumulation may occur during CRRT in critically ill patients with impaired liver function. The aim of this study was to evaluate the actual influenc...
